| Job Information | Full time Home Health RN to be liaison to two skilled nursing homes and to visit the patients discharged from those two facilities in their homes and will act as nurse liaison to the nursing homes. Nursing homes in Chicago Gold Coast and Evergreen Park. Must have reliable transportation, OASIS trained, EMR, Illinois licensed and current, and experience in home health with excellent references. Salary approximately $65,000 plus benefits based on experience. |
